Which Visa form (number/code) should I use for a 10 day tourist trip? Where can I find the form?

Frank, thanks for your earlier answer. I'm afraid I found the Japanese automated telephone service a little difficult to follow after the first few choices, so I have a few more questions:
1. Which Visa form (number/code) should I use for a 10 day tourist trip?
2. Where can I find the form? What other documents do I need to prepare?
3. Can I submit the application and supporting documents by post? If so what is the address? If not what are the opening times and address for submitting in person?
4. How many days does this usually take (Flight is booked 9 August 2012).
5. Do I have to collect the visa in person, and if so what is the address/opening times for collection?
6. How much is this visa for a UK citizen traveling from Narita to Pudong, and what payment methods are acceptable?
7. Is there anything else that I need to know in order to secure a visa before 8th August?

1. You need to fill both application form V2011A and V2011B if you will apply China visa in Japan.
2. The forms can be downloaded from this website, as well as the wensite of the embassy of China in Japan. Documents you need are your passport, photos, Entry record for Japan, your travel itinerary, return ticket and hotel reservation in China.
3. You need to submit your application in person or by someone else or a travel agency to handle this on your behalf if you do not want to go in person. Working hours: 09:00--12:00, Monday-Friday (except public holidays)
4. The regular processing time of a visa is 4 working days. They provide express and rush service as well if you need.
5. Passport can be collected by anyone with the Pick-up Form you get when submit your application. Office hours: 09:00--12:00.
6. Visa fee for a single entry China visa is Yen 4000 under regular processing. Chinese embassy only accepts cash as the mean of payment.
You can use express service if necessary, extra Yen 3000 is required.
