About:China Work Visa

To apply a work permit in China, I need proof of no crime from local US police; what form?

Is there a standardized form or procedure I need to prove I have no criminal record when I apply for a work permit in China

To obtain police clearance from your state or states of residence, you must contact the police in the state or states of the United States in which you are or were resident. There is no need for you to provide any form. So, just go ahead to the police station.
