re: photo for visa application form, names

In the photo instructions, what does "identical" mean?

For part 1.4,
Do I need to include my nickname and stage name?
Also, I was divorced almost 30 years ago - do you need my pre-divorce name?

No, there is no need to put in your nickname and stage name or pre-divorce name. You just put in your current name of your passport.
