About:Chinese Consulate in Vancouver

I left Hong Kong in Oct 1966 to immigrate to Canada. Have not renew my HK ID card since.

I plan to return to HK in Nov this year. Would like to apply for the HK Permanent Resident ID card in Van. Would like to know which office I should go to submit my application form.
Please send me the link to print the application form, and to read about the required documents I should have in order to submit the application form.

Since you have not renew your HK ID, it is invalid effect from 30 November 2008. Now, you you should make an appointment for the application at any of the Registration of Persons Offices. It is ok to apply after you enter HK (within 30 days after entry). If you apply through Chinese consulate in Vancouver, application form ROP143 is required.

1.From Internet I found 3 addr for Chinese Consulate in Vancouve . Which one should I go to submit my application- form ROP 143?
2.How do I put my left and right thumbprints on the form? Please forgive my ignorance and help me.
3.Do I need to call the Chinese Consulate in Van to make an appointment before I go?
4.If my Oct 1966 HK ID is invalid, do I need my H Sch transcript to prove my 7-year HK residency? I do not have it in Vancouver. I spent all my life in Hong Kong until I immigrate to Vancouver in Oct 1966.

1. The adress is 3380 Granville Street, Vancouver, B.C. V6H 3K3
2. You just need to put your signature on the form, no need of thumbprints;
3. No appointment is needed.
4. It does not matter that you do not have the H Sch transcript to prove my 7-year HK residency.
