1. Application form for HK SAR passport (ID845) and copy;
2. Payment form （ID853）and copy；

5. If you have already get Canadian citizenship, you need to provide your original Canadian passport and copy of the first page; original Citizenship Card and copy; original and copy of Record of Landing.
