Hi, Macy, sorry to tell you that your daughter can not share a visa with you. She needs a independant L visa, but the good news is that she can share the same passport with you. To remind you, if you two share the same passport, you need to fill in the V2011b application form. Good luck with your application!

Thank you Melissa for your kind answer... She has her own passport. What other requirements required for infant? Thank you once again.

You need to prepare for her the DSWD Travel Clearance for Minor Filipino Citizens. Her father shall execute a duly-notarized Parental Travel Permit (in a form prescribed by DSWD) certifying that he is allowing your baby to travel with you. No other document is required.

Thank you Alvin for your time... I see, it's quite different traveling to HK as I didn't bring any documents with me apart of her birth certificate.
Thank you once again Alvin for your time and kind response.
