1.You should provide the booking document for the hotel in China;
2. You have to fill in the 2.5 blank with information of your residence(s) and phone number(s) during the visit in China in a time sequence. But I heard that they seldom check about that.
3. Lastly, to remind you, this is a turism website, not the official website of Chinese Visa.
