For a China work visa you need to submit the followings:
1. Original Visa Notice from an authorized unit in China (from the Local foreign affairs office or other governments in China)
2. Employment permit issued by relevant bureau in China
3. Physical examination record for foreigners.
4. Photocopy and original passport valid for more than six (6) months and must have at least two (2) blank pages for visa purposes.
5. A completed visa application form with one white background passport picture.
