to Cairo for UN WSA celebration

We are the 2011-12 World Summit Winner of the Beijing Traditional Chinese Medicine Digital Museum of China , and the celebration will be held in Cairo , late April. We have been invited by the Chaiman of WSA Board of Directors Prof. Peter A. Bruck, who gave us an official invitation letter , and we are advised to apply a tourist visa in the Egyptian Embassy. my question is:(1) We want to apply for the visa directly from your embassy , how to get the application form? (2) How much is the fee? pay in cash or through a bank? Be kind to answer. Thank you.
