Name Discrepancy on my passport, driver's license and ITR and SSS documents

Hi,

I'm annulled and already using my maiden name again for my passport and driver's license. Since im applying for an L visa i need to present my ITR and SSS docs which apparently are still under my married name. I already did the necessary steps to amend them but they are taking a long time to take effect in the system. My flight is already on March 17, 2012 and im planning to submit my application next week.

I had my lawyer prepare an affidavit stating that my married name and maiden name pertains to one person. My lawyer also advised me to attach a copy of the annulment court order and my birth certificate.

Is this the right thing to do for my case or will there be any additional requirement?

Monica, it is the right thing to do for your case. The affidavit letter stating that your married name and maiden name pertains to one person, a copy of the annulment court order and birth certificate are enough to prove it is a same person.
