ReplyYou have to prepare the following documents:
1. A completely filled passport application form;
2. 2 color recent-taken photos of 48mm X 33mm without hat and with white or light blue background;
3. Your original passport and its photo copy;
4. photo copy of the persoanl details page of your passport;
5. USA visa;
6. Resident visa, Green card or work permit in US00Reply
