what documents are required to renew 2 years multiple entry F visa?

Dear Im holding lebanese passport , and i have residence card in belgium , my visa is expired , and i need to renew .
i use to have F visa 2 years multi entry and it expired in 11 february 2012 .
please advice what document i need to renew so i can arrange it s soon as possible .
