About:Foreign Consulates in Hong Kong

Hi I am Filipino Living in Leyte. Do I need visa to transit in Hong Kong?

do I need to fill up an immigration form for hongkong? because I will have my vacation this December to Milan Italy, and the plane will landed at Hongkong International Airport?

No, you don't need a visa. But you may asked to fill up a form at the airport if you go out of the transit area.
