About:Chinese Embassy in Philippines

Which Form V2011A (visa application form) should i use to apply for China visa for tourist?

There are two formats of the same Form V2011A circulating among travel agencies and in the various related online sites. One format has its label "Form V.2011A " on the upper left side of the form, while the other one has it on the right side. They ask for the same data but they differ in format and phrasing of questions. One has this printed at the bottom of the page: CNNYCONSULATE JUNE 2011 EDITION , and instructs the applicant to fill out the form "on a computer and print out "; the other one does not contain these phrases and instruction. Which one is the appropriate form for me to fill up? I'm a bit confused and didn't want to repeat the filling up of forms.

You need to use the format that has its label "Form V.2011A " on the upper right side; doesn't contain these phrases and instruction "CNNYCONSULATE JUNE 2011 EDITION , and instructs the applicant to fill out the form on a computer and print out "
