The visa application forms should be filled by one of his parents because he is under 18 years of age. His passport, one passport, The latest 3 months bank-statements with adequate money, Hotel reservation in China; Round-trip ticket booking are all required. He had better get an consent letter from his parents and the copy of his parents' passport, maybe they are required by the visa officer.
