About:Chinese Embassy in Japan

Where I can find a Chinese Embassy approved health check form in Japanese and Chinese?

I am moving to China this summer to work. My company has asked me to get a health check here in Japan (where I am currently a resident). I have a copy of the English/Chinese health check form from the Chinese Embassy in the UK website but I was wondering if there is a Japanese/Chinese heath check form.

There isn't a Japanese/Chinese health check form, you can use the Physical Examination Record for Foreigners Form download from the website of Chinese embassy in UK.
