About:Chinese Embasy in Ireland

Hi,

I've been asked by my employer in China to supply a medical certificate assigned by the Chinese embassy in Ireland.

What do I need to bring with me to the embassy when applying for this certificate?

Thanks in advance for your help.

You should first make the medical certificate signed by a hopistal cerificated by Legalization Office of the Irish Department of Foreign Affairs, then go to China embassy to apply for authentication.

You need to fill the Authentication Application Form, take your passport, one passport photo, the documents of medical certificate

Hi,

Thanks for your reply.

I see on the form from the embassy there is a box for a chest x-ray exam. Is this a requirement for certification or is this just in case there are other complications found in the physical examination?

Many Thanks,
Johnny
