If already a US Citizen with US Passport but would like to use the HK re-entry permit + Hong Kong ID Card to in/out Shanghai or China directly from USA, (not using the US Passport) is it OK?

No, sir, you can use your HK re-entry permit (bearing '***', '*' or 'R' symbol or locally issued Permanent Identity Cards) and HK ID to Shanghai by passing through Hong Kong immigration control only.
