The China visa application forms can be found on the top of this page, which you can either fill online or print out first and fill by hand.

You are not required visa to Macao for 30 days, but you need to apply for a visa to Mainland China. The requirements are your origianl passport, photo, application form, round-trip tickets, hotel reservation. If necessary, you may be required to provide financial certificate for travelling, as well as the residence/work visa in Cayman or else.
