A Chinese nationals should go to China embassy/consulates which holds consular jurisdiction over the state where he/she resides to submit China passport application in person, the below documents are required to submit:
Filled China Passport Application Form
Two passport photos
his/her original China passport and its photocopies
a valid US visa or foreign residents with legitimate status in the US, such as Green Card, Work Permit,I-20 Form

Mailed applications are not acceptable and will be returned.
No appointment is required
