Mr.Naveen, you are required to apply for China visa from CVASC in Mumbai. "Physical Examination Record for Foreigner" can be issued by doctors from an Indian state hospital. The photo on the Record should be stamped by the hospital.(Be noticed that the Record is valid for 6 months from issued date for visa application).
