please apply for a China visa from Chinese embassy or CVASC in your country, and most likely, you need to apply for a Tibet Entry Permit after you get tha visa.  contact the Tourism Administration of TAR (Tel: 0086-891-6834313; Fax: 0086-891-6834632) and get a visa notification form issued by it in advance.  Well, for the costs, you can search for the Train and Flight system on this website to find out it.
