Is it disrespect or considered bad manners not to use chopsticks?

No, it is not a bad manner or disrespect to use chopsticks. If you can not use chopsticks, you can ask for fork and knife.

There is usually no fork and knife at most Chinese food restaurants in China.

It isn't rude to not use chopstick but however we don't usually use forks at a Chinese restaurant . The cutlery we use are spoon, knife and chopsticks.
