You need to submit Chinese passport application in person to the Chinese consulate in Toronto, therefore, you can get the Chinese passport application form from the consulate for free and fill it at the consulate, when you go to the consulate, please take your present Chinese passport, two passport color photos, if you have gotten Canada Permanent Residence Permit for 5 years, you need to provide certificate that you don't join Canada nationality.
