About:China Student Visa

Dear

I have done my physical examination for foreinger,the format which i download from a website. later i came to know that there is one CERTIFICATION OF VARIFICATION done for the physical examination attested by the same doctor,but i could not find the format from web,can anybody help please??

dear I found both pages.my question is that is there is any verification certificate required for the Physical examination?

M.Navin, I didn't hear that there is verification certificate required for Physical Examination Record, as I know, for student visa application, Physical Examination Record for Foreigner issued by doctors from an Indian state hospital is Ok.
