I want to go back to china this month to attend my dad's birthday, and to surprise him. My last visa issued was on April 2010. I arrived there on May. I had 2 months extension there - last June and July. I left China last August 10, 2010 only. Can I apply for a visa again this month? What are the requirements? Do I need a bank certificate?

you dont need bank certificates since you have been issued a sticker visa before. the only problem and it is questionable that you will be coming back so soon after 2 extensions and they are strict to philippine passport holders. there is a chance that you will be given a lesser duaration than a 30 day one. you can try
