If your name is not included on the visa notification form which was issued to your husband, your husband has to go to Shanghai Foreign Affairs Office to apply for a visa notification form for you, then you can apply for Dependant Visa.

You need your husband to get help from his employer to process visa notification( in china) for you as dependent, but aside from this, you need to have some supporting documents such as: bank account statement(P50,000/person),invitation letter from your husband/company,residential contract( your husband or you need to show hotel booking).Hope it helps.
