Could you please send me the application form and a list of the documents needed for Hong Kong visa?

The requirements for HK visa are as follows:
1. Hong Kong Visa Application Form(https://www.travelchinaguide.com/embassy/pdf/hk-new.pdf) .
2. one recent photograph (affixed on page 1 of the application form);
3. Photocopy of your passport;
4. Photocopy of your financial standing;
5. Photocopy of proof of transportation arrangement, e.g. flight itinerary;
5. Photocopy of proof of relationship with the local sponsor (for family visit only).
