i would like to travel to china for tour but how can i get the application forms and what is the requirements

You can download the visa application form from the top of this page, the requirements for China tourist visa are:
1. Properly filled out Visa Application Form, to which one passport-size photo is attached
2. Valid passport
3. Round trip air-ticket, hotel reservation, travelers check and original invitation from an authorized Chinese travel agency.
