hellow,*where do i get the application form noeap-1?

hellow,*where do i get the application form noEAP-1? DO i have to get it from ambassy only*renewal done for how meny years?i mean passport validity issued for how long period?is it issued for 10years or 5 years? now in india renewal period is 10years for all passports either general or tatkaal,pls give me information how about in ambassy.

1. The EAP form can be downloaded from http://www.indianembassy.org.cn/DynamicContent.aspx?MenuId=61&SubMenuId=532. I get it from the official website of the Indian Embassy in China that it will take 30 days to renew a passport.
