About:Chinese Embassy in U.S.A.

I am a U.S citizen, and I need to travel to China in the middle of May. It's stated in the NY Consulate website that "first time applicants whose former nationality was Chinese, shall write down his Chinese name in the application form and submit his original Chinese passport or the photocopy of the information page of the passport," but I cannot find my Chinese passport anywhere at home. What can be done so I can go visit China?
