In March I am going to apply for student visa for Shanghai for 4 Months, however, I might study there longer. Do I have to take out med. insurance for China? In addition, do I have to fill in or submit any other med. certificate from my physician in Ukraine for the Chinese Embassy in Ukraine? Thank you

If you will stay in Shanghai for more than 6 months, you need a Foreign Medical Check at a hospital accepted by Chinese embassy in Ukraine, otherwise, you don't need the medical check, you should apply for F visa for not more than 6 months study in China.
