About:Chinese Embassy in New Zealand

I am visiting China to stay with relatives in Teinsin on the 23 sept.2009. Could you send me the forms I have to complete for a tourist visa to Maureen Wootton, Flat1/66 Flemington Ave, Queenspark , Christchurch New Zealand.
