Thanks for ur information. I already got this form but i want to know where i can do all these test. Is there any recommend hospital or diagonastic center for Chinese Embassy in Dhaka or Chittagong? Or i can do it any well known Hospital and diagonastic center in Chittagong.

You should visit the embassy's website to see if there is list of the designated hospitals where you can undergo a physical check-up. If there isn't any, you can go to the big public hospital near your domicile.
