Is there any new link to download the Visa form for Chinese Visa Application to go to Macau/Zhuhai and Hkg?

Hi,Im holding Indonesian Passport. We are planning to go to Macau/Zhuhai and Hkg in this month. We just want to have a day trip to Zhuhai from Macau.
I believe we need to have L visa to Zhuhai but I have tried to download the Visa form from "Chinese Visa Application Form Dowload Here(in PDF form) !" I got error msg The page can not be found.
Is there any new link to download or kindly send me the form to yahoo.com|wramali.
