i am a malaysian i would like to apply a student visa in china (nanjing), what document

Hi...I am a malaysian I would like to apply a  student visa in China (Nanjing), what document should I support for the applying ? Pls advise me. ( i have received a JW 202 form from the china school,but they type wrongly my name (e.g My actual name is Chek Jia Jun, but they written asCHEK JIR TUN, ana also my house address. ) can i use this JW202 form to apply the student visa.THANK YOU.tq

Probably, you can not use that form to apply for X visa since the important information is faulted.
