Currently the answer is Yes. The 15-day visa free visit for Singapore passport holders has not been resumed yet.

We are planning a free & easy tour plus visit relatives in Hainan somewhere in Oct 2023. Do we still need to apply for visa?

If you only stay in Hainan, you are eligible for the Hainan 30-day visa-free access. But you need to register with a local agency.
