About:China Visa

I fly to cleveland OH USA, transit in shanghai, I hold a Italian passpo do I have to fill up a form?

If you can enjoy the free transit in Shanghai, you don't need to fill up the visa application form, but you need to fill an Arrival/Departure Card and then apply for the free transit after arriving at Shanghai.
