Visa Requirement for a Toronto Canadian Resident

Hi, I live in Toronto - looking to enter into mainland China from Hong Kong - what is my visa requirement? Also, I am traveling as a tourist - do I need to state which hotels I'm staying in while traveling through Mainland PRC as all my hotels have not been arranged as of yet except the first hotel I will be staying at in Shanghai?

Miran, if you are in Toronto now, please apply for an L type from the CVASC office there. By the way, you don't need the entry permit for HK. To apply for the L type, please all the following materials ready, which include your passport, passport copy, application form, qualified photo, round-trip tickets and hotel reservations.
You don't have to book the hotels covering the whole trip, but you are required to provide the booking order of most hotels where you will stay.
