Tokyo to Shanghai for American

Hello I will be staying in Shanghai for 7 nights. I live on a US military base in tokyo. What else documentation do I need to bring with me to the China Embassy to get my Visa. I know I need to bring the application form and my passport, but is there anything else that I need.

Thanks

You should also prepare light tickets (for both entry and exit), hotel bookings for your stay in Shanghai, and a passport sized photo. Original and copy of your Japanese entry record or visa are also required.
